The Kansas Department on Aging (KDOA) has been notified that the Centers for Medicare and Medicaid Services (CMS) has imposed a $5,000 civil monetary penalty on Garden Valley Retirement Village, 1505 E. Spruce St., Garden City.
The fine resulted from violations which significantly and adversely affected the health, safety, nutrition or sanitation of the adult care residents of the facility. The violations were identified in a survey that was completed on July 28.
KDOA conducts the facility surveys, but the federal agency imposes the penalties.
