The Kansas Department on Aging (KDOA) has imposed a civil monetary penalty of $2,500 on Country Squire Senior Gasthaus, 2761 Trefoil Road NE, Waverly.
KDOA staff inspected the adult care home on May 25, 30, 31 and June 1, 2006, and determined that the facility had violated rules and regulations established under the adult care home licensure act and that those violations resulted in, caused or posed serious physical harm to residents in the facility.
The facility has the right to appeal the assessment.
