The Kansas Department on Aging (KDOA) has been notified that the Centers for Medicare and Medicaid Services (CMS) has imposed a $7,000 civil monetary penalty on Infinia at Smith Center Inc., 117 West 1st Street, Smith Center.
The fine resulted from violations which significantly and adversely affected the health, safety, nutrition or sanitation of the adult care residents of the facility. The violations were identified in a survey on January 4, 2006.
KDOA conducts the facility surveys, but the federal agency imposes the fine.
