The Kansas Department on Aging (KDOA) has been notified that the Centers for Medicare and Medicaid Services (CMS) has imposed a $9,000 civil monetary penalty against Medicalodge of Leavenworth, 1503 Ohio St., Leavenworth.
The fine resulted from violations which significantly and adversely affected the health, safety, nutrition or sanitation of the adult care residents of the facility. The violations were identified in a survey conducted by KDOA in on December 15, 2005.
KDOA conducts the facility surveys, but the federal agency imposes the fine.
