The Kansas Department on Aging (KDOA) has been notified that the Centers for Medicare and Medicaid Services (CMS) have imposed a $5,000 civil monetary penalty on Emerald Pointe Health and Rehabilitation Center, 109 W. Empire, Galena.
The fine resulted from violations which significantly and adversely affected the health, safety, nutrition or sanitation of the adult care residents of the facility. The violations were identified in a survey completed May 31, 2005.
KDOA conducts the facility surveys, but the federal agency imposes the fine.
