The Kansas Department on Aging (KDOA) has been notified that the Centers for Medicare and Medicaid Services have imposed a $10,000 civil monetary penalty against Deer Creek Senior Group Home West, 3314 Front Street, Topeka.
The fine resulted from violations which resulted in, caused or posed serious physical harm to a resident or residents of the facility. The violations were identified during a survey conducted Feb 22 through 28 and March 1, 2, 3, 7 and 8, 2005.
KDOA conducts the facility surveys, but the federal agency imposes the fine.
